Reporting a Stolen Indiana License Plate
If you reasonably believe that your Indiana license plate was stolen from your vehicle, come to the Porter County Sheriff’s Department, Monday through Friday (excluding holidays), 8:00 am to 4:00 pm to make a theft report.

Fee is $5.00 cash, credit/debit or money order only
Reporting a Lost, Mutilated, or Destroyed Indiana License Plate
If your Indiana license plate was lost, mutilated, or destroyed, complete the Indiana Bureau of Motor Vehicles form - Application for Duplicate or Replacement License Plate (State Form 37135). Follow the directions at the top of the form. Click the document below to download the State Form 37135.
