Reporting a Lost, Mutilated, or Destroyed Indiana
License Plate
If your Indiana license plate
was lost, mutilated, or destroyed, complete the Indiana
Bureau of Motor Vehicles form - Application for
Duplicate or Replacement License Plate (State Form
37135).

Follow the directions at the
top of the form. If the form requires an officer’s
signature, bring the completed form to the Porter County
Sheriff’s Department Monday through Friday, 8:00 am to
4:00 pm.
Reporting a
Stolen Indiana License Plate
If you
reasonably believe that your Indiana license plate was
stolen from your vehicle, come to the Porter County
Sheriff’s Department,
Monday through
Friday (excluding holidays), 8:00 am to 4:00 pm to make
a theft report.
